The safe system of work
- Plan the task and work to the method, risk assessment and any permit.
- Identify and isolate every energy source, then release or restrain stored energy.
- Verify zero energy before starting and lock off with a personal lock and tag.
- Use the right tools, access equipment and PPE for the task.
- On completion, restore guards, remove tools and confirm the machine is safe.
Permits, isolation & guarding restoration
- Higher-risk maintenance is controlled by a permit-to-work.
- Lockout/tagout keeps the machine isolated while work is in progress.
- Removed guards and panels are refitted before returning the machine to use.
- A function and safety check confirms the machine is safe before restart.
Required PPE
- PPE for the specific hazards: electrical, mechanical, chemical and thermal.
- Eye, hand, foot and body protection as the risk assessment requires.
- Respiratory protection where dust, fume or contaminants are present.
- Fall protection and rescue arrangements for work at height or in confined spaces.
Pre-work checks
- Confirm the permit, isolation and verification are complete before work.
- Check tools, access equipment and PPE are suitable and in good order.
- Make sure others know work is in progress and the area is controlled.
- Do not start until the machine is confirmed isolated and safe.
Isolation & lockout
- Isolate every energy source and lock off with a personal lock and tag.
- Release or restrain stored energy before any work begins.
- Verify zero energy by testing before reaching into the machine.
- Only the person who applied a lock removes it, once work is safely done.
What the worker is and is not authorised to do
- Carry out only the maintenance you are trained and authorised to do.
- Work within the permit and isolation in force — never beyond it.
- Do not work on electrical, pressure or confined-space systems without the specific authorisation.
- Do not return a machine to use until guards are restored and it is verified safe.
What to do if something is wrong
- Stop if isolation cannot be achieved or verified, or conditions change.
- Report defects, missing guards and any failure of isolation.
- Leave a machine clearly out of use until maintenance is complete and checked.
- Report any injury, near miss or unsafe condition.
Training & competence
Reading this page does not make a worker competent or authorised to carry out maintenance. Maintenance requires trade or task training, instruction in the employer’s permit and isolation systems, and an assessment of competence.
Electrical work, pressure systems, confined spaces and other specialist maintenance legally require formal training, recognised qualifications and specific authorisation, and may be restricted to named competent persons. This page provides none of these.
