What this toolbox talk covers
This is a brief outline a supervisor can use to talk through lockout/tagout (LOTO) — isolating a machine’s energy, locking it off and proving it is safe before anyone works on it.
It is an awareness prompt, not a training course. Adapt it to the actual machines and your employer’s isolation procedures, and always follow them and the risk assessment.
Why it matters
- Machines that start unexpectedly during maintenance cause severe injuries.
- Stored energy can move parts even after the power is switched off.
- Locking off ensures the person working on the machine controls when it restarts.
- A short reminder reinforces that switching off alone is not enough.

- A machine restarting while someone’s hands are inside it.

- Removing the wrong lock or skipping verification before work.
Key safe-behaviour points
- Identify every energy source before starting — electrical, pneumatic, hydraulic and more.
- Isolate, lock off and apply a tag that says who has locked it and why.
- Release or restrain any stored energy before work begins.
- Verify the isolation by trying to start the machine, then return controls to off.
- Each person working applies their own lock; only they remove it.
- Never remove someone else’s lock or work under another person’s isolation.
Dos and don’ts
- Do follow the machine’s isolation procedure step by step.
- Do check the machine is truly dead before putting hands inside.
- Don’t rely on a stop button or a switched-off control as isolation.
- Don’t remove a lock or tag that is not yours, for any reason.
What to do if something is wrong
If a machine cannot be properly isolated, has no clear procedure, or you find a lock or tag you cannot account for, stop and tell a supervisor — do not start work or remove the lock.
Report any case where a machine could have restarted unexpectedly so the procedure can be reviewed.

- How many energy sources does this machine actually have?
- Where might stored energy be hiding after we switch it off?
- How do we prove the machine is dead before reaching in?
- What do we do if we find a lock and don’t know whose it is?
Real-world reminders
- A stop button is not an isolation — it can be overridden or reset.
- Air, hydraulic and gravity energy can move parts with no power on.
- “It’s only a quick job” is when shortcuts and serious injuries happen.
- Your lock is your guarantee that no one else can start the machine.

- Record any isolation difficulties or missing procedures raised.

Common mistakes to avoid
- Treating a switched-off control as a safe isolation.
- Isolating one energy source and forgetting another.
- Skipping verification because the machine “must be off”.
- Removing another person’s lock to get the job moving.
Who is responsible — awareness, not training
The employer remains responsible for isolation procedures, lockout/tagout training and authorisation, the risk assessment and records. A toolbox talk raises awareness; it does not train, test or authorise anyone to isolate or work on machinery, and the deliverer should be competent.
