What this record is for
A PPE issue record gives you a tidy way to keep track of which protective equipment has been issued to which worker, on what date, and that they were shown how to use, store and check it. It is a free educational starting point you can adapt to the PPE your workplace uses.
It does not decide what PPE is needed, replace the risk assessment that identifies it, or replace the training and instruction workers must receive. PPE is a last line of defence after other controls. Employers remain responsible for selecting suitable PPE, providing it, and following the applicable law and the official authority’s guidance.
What a good version covers
- Worker name and role, and the date PPE was issued.
- The items issued (for example eye, hearing, hand, foot, head or respiratory protection).
- Size, type or standard where it matters to the fit and the hazard.
- Confirmation that instruction on use, storage and inspection was given.
- Any fit check carried out where the equipment requires one.
- Replacement or reissue dates and a note when items are returned.
- A signature from the worker acknowledging receipt where used.
How to use and adapt it
- Base the items on what the risk assessment identifies as necessary — not a generic list.
- Issue PPE alongside instruction on how to wear, store, check and replace it.
- Record reissue when items wear out or no longer protect.
- Keep the record with the worker’s other safety paperwork.
- Remember PPE supplements safe systems of work; it does not replace them.
- Defer the choice and standard of PPE to your risk assessment and the official authority.
Common mistakes
- Issuing PPE without first assessing what protection is actually needed.
- Logging the handover but giving no instruction on use, fit and storage.
- Treating PPE as the first control rather than the last line of defence.
- Never reissuing damaged or worn-out equipment.
- Assuming the record itself makes anyone competent or compliant.
