What an appraisal form is and its purpose
An appraisal form is the written template that captures the outcome of a performance assessment. It records the objectives reviewed, the manager’s and employee’s comments, any ratings used and the development plan agreed for the period ahead.
Its purpose is to make assessments consistent and documented. For the employer it provides a comparable record across people and cycles; for the employee it sets out the feedback in writing, with space to add their own view and agree next steps.
